We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. The Case Manager provides comprehensive case management, crisis services, and care coordination to children, families and adults served by Amoskeag Health. The Case Manager works directly within the medical clinic as part of a multi-disciplinary team, providing ongoing case management services on-site, in the community, and in the home.
