Baylor Scott & White Hospice is a collaborative, interdisciplinary team of professionals providing end of life care for patients and support for families in their homes.Nurses, Home Health Aides, Case workers, Chaplains and Volunteers all work together to provide professional compassionate care.Nurses have autonomy, no office- start and end from home, limited call, no weekends (except weekend positions)12-16 patients per RNwith anticipated 2-4 visits per day. They craft care plans and track patient progress. What You'll Do Develop, implement, and revise care plan for patient outcomes. You will evaluate the quality and effectiveness of nursing practice, including delicate indicators. You will support evidence-based practice changes through research. Quality/performance improvement initiatives are part of your role. You must create a safe, helpful environment for patients, families, and team members. Build therapeutic relationships, manage pain, and set boundaries to succeed in this role. Your role will encompass respecting and incorporating a patient's and family's unique differences into an individualized plan of care. Maintain patient and family privacy, autonomy, dignity, and rights. Create a plan reflecting their values and goals, resolving ethical or clinical issues. You will help educate patients, families, and team members to make informed choices about healthcare and treatments. You will work with others to create a care plan, being open and sensitive to everyone's ideas. What Will Make You Successful In The Role A deep understanding of medical ethics and patient privacy rights is essential. Effective communication underpins all aspects of nursing. Being sociable and having the skills to interact with a variety of people will boost your performance in this role. Observing changes in patients' medical conditions and effectively relaying this information to other nursing staff and care providers is vital. Provide patient-centered care responsibly, adjusting to each patient's needs and experiences with respect and consideration. Computer skills are important. Obtain Basic Life Support certification within 30 days of hire.
