Summary : Under the supervision of the Director-Care Management Services, the Peer Community Health Worker (CHW) will provide support at outreach and participate in community events. CHWs will also assist with data entry, outreach, education, and other administrative tasks associated with the project. At events, CHWs will provide education and assistance with registration for the COVID-19 vaccine. Other duties would occur to support the project, such as data entry of registration forms, quality assurance, and other administrative tasks as assigned. Essential Functions: Reasonable Accommodations Statement To perform this job successfully, a Peer Community Health Worker (CHW) must be able to perform each essential duty satisfactorily. Reasonable Accommodations may be made to enable qualified individuals with mental health illness and/or substance use disorder to perform the essential functions. Reports to the Director-Care Management Services or designee. Establishes and maintains good working relationships with staff from other agencies and staff from Ibero programs. Facilitates various outreach events, workshops, and activities for clients and families. Maintains accurate records and progress notes. Effectively engages and communicates with other Program staff to integrate client services. Coordinate's activities and maintains appropriate schedules. Conducts community outreach. Conduct door-to-door outreach. Perform vaccine registration. Wear appropriate PPE and follow infection control guidelines at event sites. Complete data entry outside (as assigned based on CHW preference). Work and collaborate with community partners. Maintain high-level agency policies and procedures framework to work a flexible schedule. All other duties as assigned by the supervisor. Authority: The employee exercises considerable initiative in setting priorities within the framework of the agency's policies and procedures. The individual must be resourceful and able to display initiative and understanding of interpersonal relations. Ethical - Ability to demonstrate professionalism conforming to values and accepted standards. Interpersonal-Ability/desire to understand others' attitudes/interests/needs/nonverbal behavior, listening skills, and understanding strengths/limitations of others. Reliability - The trait of being dependable and trustworthy. Teamwork - Ability to work cooperatively and effectively with others, being part of a formal/informal team, solicits input, credits others, may require group conflict resolution skills and/or motivating others. Communication, Oral - Ability to communicate effectively using the spoken word. Communication, Written - Ability to communicate in writing clearly and concisely. Skills and Abilities: NYS Certified Peer Specialist Provisional or Permanent Certification preferred. High school diploma or equivalency required. A resident of Rochester with good knowledge of the city's communities and resources. Experience in public health, healthcare, customer service, or a related field preferred. Valid NYS driver's license and able to provide own transportation. Bilingual in English/Spanish preferred. A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and Physical Demands: The position does require occasional standing, squatting, lifting of up to approximately 40 lbs.
