Job Summary: The LPN performs a variety of nursing duties in the assisted living, complies with state rules & regulations, and follows policies & procedures of the assisted living. Manages the medications given to residents in assisted living & memory care and maintains the medication carts. Provides education to staff about medication administration and provides wound care to residents as need. This position reports to the Assisted Living DON. Essential Functions: Ensures that unlicensed staff members perform their duties and responsibilities in a timely, quality manner, and reports problems to RN. Initiates emergency procedures for clients and reports to RN. Reports changes in client condition or other concerns about clients to the RN in a timely manner. Reviews client care plans developed by the RN and consults with RN about client conditions and their treatments. Enters incident reports to Eldermark and assists with follow-up. Ensures accuracy and promptness in transcribing physician orders. Assists in implementing physician orders. Carries out proper charting practices and documentation procedures. Assists with startup of services for new clients, transfers to settings such as hospitals, nursing homes or assisted living and discharges as requested by RN supervisor. Assembles and operates equipment such as oxygen suppliers and other equipment used for client care. Orders, sets up and administers medications. Does monthly cycle fill and follows up with the pharmacy. Attends required meetings, in-service training, and conferences. Assists the RN in the monitoring, mentoring, orienting, and evaluation of the job performance of unlicensed staff. Performs delegated tasks and other duties as assigned by the RN. Physical Demands: While performing duties of this job, employee is regularly required to use hands and fingers to examine clients, handle or feel objects, tools, or controls. In addition, the job requires walking, bending, stooping, pushing, pulling, kneeling, reaching overhead and frequently lift and/ or move up to 10 pounds and regularly lift and/or move up to 50 pounds. Occasionally lift and/or move up to 100 pounds. Push up to 75 pounds. Position Type/Expected Hours of Work: Full Time, M-F 8am-4:30pm flexible times available, occasional weekends, evenings, and holidays. Monthly on-call phone rotation. Minimum Qualifications: Must be registered and licensed with the State of Minnesota as an LPN and must maintain current unencumbered license with the Board of Nursing.
